Subacute Care

Subacute care is for medically stable patients who require nursing, rehabilitative care and more intensive, technologically advanced therapies than can be given at home or on an outpatient basis. In many cases, subacute care is a substitute for traditional hospital care. Physicians direct and coordinate care with highly trained professionals who form an interdisciplinary care team. Team members include:

  • Board Certified Physicians
  • Case Managers
  • Registered Nurses
  • Physical Therapists
  • Occupational Therapists
  • Speech/Language Pathologists
  • Audiologists
  • Social Workers/Discharge Planners
  • Dieticians
  • Pastoral Care
  • Recreational Therapists

When compared to a general hospital setting, our subacute units offer patients a comforting environment designed and managed to de-institutionalize medical care. A high staff-to-patient ratio means that patients benefit from frequent interaction with and accessibility to staff. The result: recovery is accelerated as patients respond more positively to therapies and treatments. We also provide patient and family support and education at every opportunity.

ACCN facilities specialize in providing subacute services for senior adults. Our services include, but are not limited, to the following programs:

  • General rehabilitation and cardiac rehabilitation
  • General medicine
  • Orthopedics
  • Neurology
  • Surgical recovery
  • Wound management
  • Respiratory
  • Swallowing Programs
