     Schnurmacher is a 245-bed facility in White Plains, affiliated with Beth Abraham Health Services in partnership with Beth Israel Medical Health Care System, providing both long-term residential care and short-term subacute rehabilitation and medical care. Because Schnurmacher is part of the Beth Abraham network, patients who are discharged have access to Beth Abraham's community programs in Westchester, such as adult day health care and CCM.

 

Residential Programs

Residential Health Care: Long-term medical and skilled nursing care and physical rehabilitation for a wide range of chronic illnesses and disabilities.

Subacute Rehabilitation: Physical and occupational therapy and speech, hearing and swallowing services for patients needing extended rehabilitation after a hospital stay to prepare them to return home.

Subacute Medical: Short term hospitalization as an alternative to care in an acute hospital, tailored to the needs of patients who require medical and nursing services, but who do not need the complexity of services associated with an acute-care hospital. Patients recovering from pneumonia or needing post-surgical wound care, chemotherapy or IV antibiotic therapy can benefit from care in this environment.

Community Programs

Patients who are discharged from Schnurmacher can receive continued support through Beth Abraham Health Services adult day health care and CCM programs in Westchester.

Adult Day Health Care: Medical and rehabilitation services, along with socialization and recreational activities, meals and transportation to and from the center. For further information call 914-683-1600

CCM: A PACE site providing a full range of medical and health-related services, at the CCM center and in the home, for frail elderly and chronically ill individuals who would otherwise need nursing home care. For further information call 914-289-0400.
