Set on a scenic, 137-acre country campus, Field Home–Holy Comforter is a 200 bed, non-profit, non-sectarian skilled nursing facility backed by a 120-year history of caring. Rehabilitation

Newly expanded short-term, long-term and respite programs with full physical, occupational and speech/language therapist staff · Helps patients reach top functioning levels for diverse needs including Alzheimer's, stroke, post-operative reconditioning, joint replacement, neurological disorders, general aging related disability, trauma, amputation, cardiac care, head injury and other conditions. · Specialized Restorative Nursing team helps patients apply rehabilitation progress to daily life. · Dedicated rehabilitation beds are available.

Respite Care

From 2-30 consecutive days to relieve care givers at home. · First NYS-approved Respite Program at a Northern Westchester skilled nursing facility.

Hospice Care

Through a nursing home partnership with Hospice of Northern Westchester and Putnam, for patients who can no longer be cared for at home.

As part of a team of dedicated, highly qualified professionals, our R.N.’s, L.P.N.’s and certified nursing assistants are committed to assuring residents of the continued opportunity to exercise choices in daily living while maintaining optimal independence in a safe, supportive, nurturing environment.

Care Planning Team

An interdisciplinary, holistic team of dietary, nursing, recreation, rehabilitation and social services professionals works in connection with patients and families to develop care plans sensitive to individual preferences and capabilities. Its mission is to help each patient achieve optimal independence, comfort and self-determination.

Recreation

Varied 7-day-a-week professional-led program · Community outings to restaurants, shopping, etc. in handicap-adapted bus. · Barbecues on patio overlooking playground · Music, art, news, intergenerational activities with Field Child Care, games, dementia programs, exercise, cooking, holiday events, etc. · Varied faith services and pastoral visits · Resident Council for input into programs · On-site styling salon · Community volunteers for supplemental visiting and programs.

Social Services

Each patient has an assigned professional social worker to assist adjustment and social well-being, guide admissions and discharges and advise on individual concerns.

Dietary

Four-week menu cycle, monitored by full-time registered nutritionists, fulfills special diet needs and is sensitive to individual preferences.

Medical

A licensed physician oversees care. Residents may choose a private or staff physician for care.

Other Services

On-site dentistry, podiatry, eye care, audiology, and psychiatry visits.

Living Environment

Private and semi-private rooms, around smaller, more personalized nursing units; Special areas for rehabilitation, dementia, and the more physically able. Inviting, scenic outdoor areas.
