The CNR Health Care Network is a not-for-profit health care organization which for 22 years has provided high quality, compassionate care to under-served populations.
Concerned with the multiplicity of chronic diseases suffered by the poor and minorities and observing major gaps in the services available to them, CNR created programs of every kind to serve them.
It is CNR’S experience that most chronic illnesses can be managed well with high quality primary care designed to meet individual needs and reduce hospitalization. CNR Health Care Network serves communities in Brooklyn and Queens through the following programs:
In its Long Term Home Health Care Program, CNR brings all necessary skilled nursing, personal care, social work and therapeutic services into the home enabling even very ill patients to avoid hospitalization and go on living in familiar surroundings. Interdisciplinary teams in a speical HIV/AIDS Home Care Program provide comprehensivbe health services, plus emotional support, to people suffering from HIV/AIDS and secondary infections.
The Center for Nursing and Rehabilitation, our JCAHO-accredited, 320-bed skilled nursing facility, is the first nursing home in New York City organized on the "neighborhood" model which focuses on residents' preferences rather than management's convenience. Living in a "neighborhood" with staff that includes social workers and recreational therapists as well as nurses, residents have greater independence in matters of daily living and particpate in decisions about how their "neighborhood" will conduct itself.
The Center for Nursing and Rehabilitation also houses a CARF-accredited Short Term, In-patient Rehabilitation and Subacute Care program.
Providing in-home palliative care, social services, and pastoral counseling, CNR’s Prospect Hospice eases the suffering of the terminally ill. The Network’ s Prospect Home Care, Inc., a licensed home care agency, serves clients in all five boroughs of New York City and in Nassau County, with nursing and support services.
Adult Day Health Care Programs, also in Queens and Brooklyn, help adults who are frail, disabled by chronic illness, recovering from heart attack, stroke, accidents, etc., to improve their health and go on living in the community.
With activities especially designed to meet the needs of those who have Alzheimer’s disease, CNR’s Alzheimer’s Adult Day Care Programs in Brooklyn and Queens keep their clients physically and mentally active and provide respite for their care-givers.
