- Many facilities are JCAHO accredited
Access to a Broad Continuum of Continuing Care Services
- Facilities offer a wide range of services, including; skilled nursing, inpatient and outpatient rehabilitation, medically complex subacute care, ventilator management, AIDS, pediatric care and adult chronic care.
Information Access
- The ACCN is implementing a system to provide timely and extensive information for clinical and administrative coordination across the network. The system will be responsive to all accreditation bodies including HEDIS, NCQA and JCAHO.
Centralized Credentialing & Contracting
- The ACCN acts as a contract liaison for all 12 member facilities. We provide centralized credentialing to assure the documentation submitted to MCOs is organized and submitted in a timely fashion.
Managed Care Orientations
- The ACCN coordinates and facilitates orientations with managed care organizations to ensure the member facilities are properly educated about the policies and procedure of the contracted MCOs.
Coordination of Care
- Member organizations offer focused care coordination and case management to ensure appropriateness of care. Each facility monitors patient outcomes. Efforts to establish network-wide standards are in process.
Exclusively Comprised of Not-For-Profit Organizations
- The 10 member organizations, representing 12 facilities are all not for profit.
Administrative Ease
- Centralized Information System provides bed availability information at any of the 12 facilities with just one phone call.
Wide Geographical Coverage
- The Alliance Continuing Care Network has 18 facilities located across the Greater New York Metropolitan Area; Brooklyn, Bronx, Queens, Manhattan, Nassau, Suffolk and Westchester Counties.

- We utilize a Team Quality Management (TQM) approach to problem solving. The member organizations have joined together to provide quality efficient health care services through sharing and integrating best practices.
